Casey Industrial currently offers a choice of two PPO plans through Anthem Blue Cross & Blue Shield. These plans allow employees to select the right plan at the right cost for their needs. Anthem Blue Cross and Blue Shield boasts one of the largest networks in the United States enabling participants to access health care nearly anywhere. Employees and families are eligible to receive health plan benefits subject to plan qualifications at a very low cost to the employee.
MetLife Dental offers a traditional dental plan which provides preventive care covered at 100% and levels of payment for other eligible dental expenses. MetLife has no network requirements.
Through VSP Casey employees can save significantly on their vision needs. VSP’s WellVision Exam® focuses on your eye health and overall wellness with a comprehensive exam at a $10.00 copay every calendar year, prescription glasses with a $25.00 co pay which covers certain lenses every calendar year, and frames every other calendar year with a $130.00 allowance for frame of your choice. There is also coverage for contact lenses.
IRS 125 Cafeteria Plan
Casey Industrial realizes the importance of helping its employees prepare for special circumstances or address special needs. In order to do this, we offer a 125 Cafeteria plan including Medical Expense Reimbursement, Dependant Care and the option to have pre tax insurance deductions. The plan allows employees to take pretax deductions from their payroll check in order to fund an account which may be used for additional qualified health expenses, dependent health care expenses, dependent care, health care premiums or any other allowable expense, as deemed by the IRS.
The current program provides a $10,000 life insurance policy through the group insurance plan with MetLife. All employees enrolled in the health insurance program are eligible. In addition Casey Industrial offers term life insurance at affordable group rates to employees and their families.
Short Term & Long Term Disability Insurance
Voluntary Short Term Disability insurance is offered in increments up to the amount of $1,000 per week to all employees enrolled in the insurance plan through MetLife. Long Term Disability insurance is provided by Casey for eligible employees.
Accidental Death and Dismemberment
The AD&D plan is designed to provide a lump-sum benefit of up to $10,000 in the event of accidental death or dismemberment. Casey provides this through MetLife to all employees enrolled in the insurance plan.
All Casey employees and their dependents have access to an employee assistance program (EAP) through Aetna Resources For Living (formerly Horizon Behavioral Services.) The EAP provides unlimited phone access to master’s level counselors and up to three face-to-face counseling sessions per issue for the employee and his or her dependents along with several other complementary services such as will preparation. There is no fee to the employee to access these services.
401(K) Retirement Plan
Casey Industrial offers an elective profit sharing plan with a 401K feature to employees. The 401K program is to help Casey employees plan for retirement. To be eligible you must meet all of the requirements. Currently, Casey Industrial matches $.50 for every dollar invested up to 4% of an employee’s annual pay.
